A question about merging excel information into a (word Doc)

Hi! I have a excel worksheet with this info in the first 4 columns:
 
Last_Name,  First_Name,  CERTIFICATE_NAME,  Course
 
I am printing out Course certificates for my school and have it working that this information is being merged into the corresponding spots on the certificates.
If the students get more than one course certificate it prints out another certificate.
I really
want it to print out one certifiacte with all the courses listed on one certificate(word doc) not mutliple certificates(word doc)
 
My question is that when a student has more than one certifcate (they have got a certifacte in more than one course) I want it to place that course name on the same certificate.
 
I am thinking that I will need an IF statement that checks if the next Course has the same Last_Name as the last Course then I would add it to the same certificate.
I believe I would need another/many course fields on my word document(certificate).
Not sure
how to do this or if its even possible.
